School Registration Steps
School Mint Registration
Parents please register using the link below. This is the first step in the registration process.
Required Registration Documents
Families may upload the required registration documents listed below when completing their online Registration Forms. The online registration forms are available if the seat offered is accepted through the School Mint Platform at https://genesisinnovationacademy.schoolmint.com/login. The required documents may also be faxed to 470-481-0608.
- A certified copy of a birth certificate, certified hospital issued birth record or birth certificate
- Copy of Parent/Legal Guardian Identification
- State Driver’s License
- State Issued ID
- Passport
- Copy of child’s social security card or waiver (Click here for Waiver Form)
- Copy of Certificate of Immunization Form 3231
- Georgia law requires students to immunize against measles, mumps, polio, rubella, whooping cough, tetanus, diphtheria, hepatitis B, haemophiles influenza, varicella, and meningitis.
- Immunization information must be on the proper Georgia Department of Public Health Form 3231.
- A notarized waiver for medical or religious exemption of immunization (Click here for Affidavit of Religious Objection to Immunization).
- Copy of last GA Form 3300 or if new to Georgia schools you must provide a new form:
- Screenings:
- Vision
- Hearing
- Dental
- Nutrition
- The Health Department or a licensed Georgia physician must sign this Georgia Department of Public Health Form 3300.
- Screenings:
- Proof of Residency
- One documented proof of where the student primarily resides and that this dwelling is within the state of Georgia.
- Home Mortgage Bill; or Home Sale Contract; or Current Apartment/House Lease; or Section Eight/HUD Housing Document; or Current HOA Bill; or A current year tax bill or assessment
- One documented proof of where the student primarily resides and that this dwelling is within the state of Georgia.
- Academic Records
- If possible, please bring most recent report card, withdrawal form from previous school and/or transcripts. These are essential for proper placement of students. Students in grades 6-8 also will need copies of any discipline records.
- Copy of Guardianship or Custody Decree (if applicable)
- Copy of current IEP (if applicable)
- Copy of current 504 (if applicable)
- Gifted Eligibility form (if applicable)
