All athletes are required to complete preparticipation physical evaluation forms (see Sports section, below).
Clubs and Activities
Genesis believes that extra-curricular opportunities can support core academic instruction, can serve to build character, and help scholars pursue life-affirming passions. Our offerings change from year-to-year based on interest of scholars, faculty, and staff. Parents can co-sponsor clubs with a staff member. Club offerings have included the following:

Athletic Forms
Form Directions:
- There are two forms in the attachment:
- Health History Form – Please fill out with your scholar
- Physical Evaluation form – Please have your scholar’s pediatrician fill out this form. Or, you can go to local clinics or urgent care facilities to have the form completed.

- Piedmont Urgent Care – Cost: $38
- CVS Minute-Clinics
- Walgreens-Piedmont Quick-Care Clinics
- Concentra Urgent Care/Clinics
- The Little Clinic (Kroger, Glenwood Location) – Cost: $29.00
- The Whitefoord Clinic (Tues., Thurs., at MLK Jr. Middle School) –Cost: $25.00; will accept insurance
